At Emory Healthcare, we are seeking a skilled Oncology Nurse Navigator to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and navigation through the complex healthcare system.
Key Responsibilities:- Ensure regular, proactive touchpoints with patients at every major care transition to provide consistent support and guidance throughout their oncology journey.
- Collaborate closely with the multidisciplinary team to receive referrals from various pathways and integrate the navigation team's efforts to address patient-specific barriers, ensuring seamless care coordination and enhanced patient outcomes.
- Offer education and support to patients and their caregivers about their treatment plans, including pre/post-operative care, chemotherapy, immunotherapy, and radiation therapy, and manage home symptom care as needed.
- Identify and eliminate potential barriers to patient care, such as logistical issues, insurance problems, or knowledge gaps, and work to eliminate these barriers to facilitate timely and effective treatment.
- Participate in department-level quality improvement projects and initiatives to enhance patient care and outcomes, utilizing evidence-based practices and data-driven decision-making.
- Bachelor's degree in Nursing required and Graduate of an accredited school of nursing.
- 3 years within oncology, with at least 1 year in assigned disease group.
- Active Georgia (GA) nursing license, eligible for licensure in GA.
