Job Summary

Concept Rehab is seeking a highly skilled and compassionate Physical Therapist to join our team. As a Physical Therapist at Concept Rehab, you will have the opportunity to make a meaningful difference in the lives of our patients and contribute to our mission of providing exceptional rehabilitative care.

Key Responsibilities
  • Evaluate patients for rehabilitation services and develop effective treatment plans to promote optimal recovery and function.
  • Provide skilled and compassionate physical therapy services to patients, including evaluation, treatment, and education.
  • Communicate effectively with patients, families, physicians, and other healthcare team members to ensure seamless care coordination.
  • Supervise and mentor licensed physical therapy assistants to ensure high-quality patient care.
  • Participate in interdisciplinary team meetings and contribute to the development of comprehensive patient care plans.
  • Meet productivity standards and maintain accurate and timely documentation of patient care.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist in the state(s) where services are being provided.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to exercise independent judgment and discretion in providing patient care.
  • Strong problem-solving and analytical skills, with the ability to think critically and make sound decisions.
