Job Summary

The Physical Therapist provides high-quality patient care and services, working collaboratively with families, physicians, and other healthcare team members to achieve optimal patient outcomes.

Key Responsibilities
  • Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
  • Develop and implement effective treatment plans to meet patient needs.
  • Treat patients according to the treatment plan of care, utilizing evidence-based practices and techniques.
  • Supervise and mentor Licensed Physical Therapy Assistants in the delivery of clinical services.
  • Communicate patient progress, problems, and plans with physicians, clinical managers, and other healthcare team members.
  • Participate in Plan of Care Meetings and Interdisciplinary Team Meetings to ensure comprehensive patient care.
  • Achieve or exceed minimum productivity standards established by Concept Rehab.
  • Participate in in-service training programs for facility staff to enhance patient care and services.
  • Record treatment notes, progress notes, and complete daily billing of services according to policy and procedure.
  • Instruct patient families and facility staff in maintenance programs to promote patient independence.
  • Participate in discharge planning to ensure seamless patient transitions.
  • Recommend equipment to facilitate improved delivery of clinical services.
  • Report equipment problems to facility personnel.
  • Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs.
  • Consistently portray the mission, vision, core values, and professional image of Concept Rehab.
Requirements
  • Bachelor's, Master's, or Doctor of Physical Therapy degree.
  • Licensed as a Physical Therapist in the state(s) where services are being provided.
