Case Manager for Vulnerable Populations

We are seeking a dedicated and compassionate Case Manager/Program Coordinator to join our team at Haitian Centers Council Inc. As a key member of our staff, you will play a vital role in supporting individuals from vulnerable populations, addressing their healthcare needs, and promoting overall well-being.
About the RoleThis position offers an exciting opportunity to work with a dedicated team, delivering high-quality services to individuals from vulnerable populations. As a Case Manager/Program Coordinator, you will be responsible for conducting comprehensive assessments, delivering evidence-based interventions, and providing guidance and support to clients. Your excellent communication and interpersonal skills will enable you to effectively coordinate care among various healthcare providers and services.
Key Responsibilities- HIV/HCV Testing and Risk Assessments: Conduct HIV/HCV testing and risk assessments for priority population members, ensuring they receive necessary health services.
- Marketing Campaigns: Collaborate with the social media team to develop marketing campaigns targeting priority population groups, promoting awareness of available services.
- Referrals and Navigation: Provide referrals for STI testing, link clients lost to care to HIV primary care services, and assist clients in navigating the healthcare system.
- Client Support: Offer ongoing HIV navigation services to a minimum of 75 clients per contract year, advocate for clients' needs and rights, and educate them about their health conditions and treatment options.
- Care Coordination: Coordinate care among various healthcare providers and services, ensuring seamless transitions and continuity of care for patients.
- Education: Associate Degree or higher in a relevant field, accompanied by equivalent practical experience.
- Qualifications: Minimum of three years of similar work experience, facilitating groups and performing outreach/community engagement activities, certified in Rapid HIV/HCV Testing, and previous experience delivering evidence-based interventions.
- Skills: Excellent communication and interpersonal skills, strong organizational and time management skills, proficient with Microsoft Office Suite, and knowledge of citywide resources and referral processes for LGBTQ+ individuals.
The estimated salary for this position is $60,000 - $70,000 per annum, commensurate with experience. We offer a competitive benefits package, including health insurance, retirement savings plan, and paid time off.
