Case Manager, Housing Program Specialist

Our services reach over 73,000 individuals and families, including almost 13,000 youth. We serve a diverse cross-section of New Yorkers, with a focus on those who are most vulnerable, including new mothers, job seekers, immigrants, and refugees.
About the Scattered Site Housing ProgramThe Scattered Site Housing Program (SSHP) is a community-based, permanent supportive housing program for HIV/AIDS Services Administration (HASA) clients. Our program provides all-inclusive housing services, case management, and financial assistance with utility bills.
We offer 75% single-unit and 25% family-unit apartments, with both case management and housing maintenance services provided. Clients with income other than Public Assistance must pay 30% of their household's income as rent.
Job DescriptionWe are seeking a highly motivated Case Manager to join our team. The successful candidate will provide comprehensive case management services to clients in our Scattered Site Housing Program.
Responsibilities include:
- Providing contract-funded activities, including intakes, assessments, service plans, reassessments, and referrals
- Ensuring clients maintain primary medical care and adhere to medication treatment
- Connecting clients to needed services, such as medical, mental health, substance use treatment, and legal services
- Monitoring clients' progress toward goals via regular telephone contact and/or face-to-face visits
- Functioning as client liaison/advocate with outside organizations
- Escorting clients to appointments and assisting with benefit applications
- Maintaining accurate client records and reporting repair issues
Requirements:
- Bachelor's degree or associate degree/high school diploma/GED with at least 4 years of experience in case management or a related field
- Two years of relevant experience, preferably in a mid-sized organization working with persons with histories of homelessness, mental illness, and/or substance abuse
We offer a competitive salary of $48,000 annually, commensurate with experience. Benefits include health insurance, dental insurance, 403(b) retirement plan, paid time off, and paid holidays.
