A Path of Care Hospice is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to our patients in their homes.

About the Role

As a Hospice LPN, you will work under the direction of the RN case manager to provide services according to the plan of care. You will continually assess the needs of your patients and report any signs of abuse or changes in their health. You will also counsel patients and their families regarding advance directive issues and maintain accurate patient records.

Responsibilities
  • Provide skilled care to patients in their homes
  • Assess patient needs and report changes in health
  • Counsel patients and families regarding advance directive issues
  • Maintain accurate patient records
  • Participate in the on-call rotation and assist with discharge planning
Requirements
  • Oklahoma LPN license
  • 1 year of clinical experience
  • Ability to care for patients and families faced with terminal illnesses
  • Valid Oklahoma driver's license, reliable transportation, and vehicle insurance
