Job Overview

We are seeking a highly skilled and compassionate LPN to join our team as an LPN Team Float. In this role, you will provide quality nursing care to patients in their end-of-life journey.

About the Role

The LPN Team Float is a full-time position working Monday through Friday from 8:00 AM to 5:00 PM. As an LPN on our team, you will be responsible for delivering exceptional patient care while collaborating with our interdisciplinary team.

Essential Duties and Responsibilities

The LPN Team Float provides quality nursing care to patients in a manner that meets the patient and family needs working under the direction and supervision of an RN Care Manager. Key responsibilities include:

  • Assessing patient needs and developing individualized care plans
  • Providing direct patient care, including medication administration, wound care, and vital sign monitoring
  • Collaborating with healthcare providers and family members to ensure comprehensive care
Requirements and Qualifications

To be successful in this role, you must possess the following qualifications:

  • A valid Registered LPN license in the State of Ohio
  • At least one year of previous nurse experience
  • CPR certification
  • Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment
  • Computer skills sufficient to properly document services and care
  • Ability to drive during daytime, nighttime, or inclement weather
  • Valid Driver's License with Safe Driving Record
