LPN Lead Care Professional Position

We have an exciting opportunity for an LPN to join our team as an LPN Team Float. This is a full-time position working Monday through Friday from 8:00 AM to 5:00 PM. The ideal candidate will possess strong communication skills, a compassionate bedside manner, and the ability to work effectively in a fast-paced environment.
About the RoleIn this role, you will be responsible for providing quality nursing care to patients in their end-of-life journey. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Key ResponsibilitiesThe LPN Team Float is responsible for:
- Conducting initial assessments and developing comprehensive care plans
- Providing direct patient care, including medication administration, wound care, and vital sign monitoring
- Collaborating with healthcare providers and family members to ensure comprehensive care
To be successful in this role, you must possess the following qualifications:
- A valid Registered LPN license in the State of Ohio
- At least one year of previous nurse experience
- CPR certification
- Certification in Hospice & Palliative Medicine (CHLPN) preferred; required within two years of employment
- Computer skills sufficient to properly document services and care
- Ability to drive during daytime, nighttime, or inclement weather
- Valid Driver's License with Safe Driving Record
