Licensed Practical Nurse Home Health Specialist

Maxim Healthcare Services is seeking a skilled Licensed Practical Nurse (LPN) to deliver high-quality patient care in a home health setting.
The ideal candidate will possess strong clinical skills and the ability to work independently under the supervision of a Registered Nurse (RN).
Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Perform focused interviews to identify specific patient needs.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
Qualifications:
- Current Licensed Practical Nurse (LPN) License in the state in which the LPN practices.
- Current Health Certificate (per facility).
- Current PPD or Chest X-Ray.
- Current BLS card.
- One year prior LPN experience preferred.
