Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Registered Nurse. As a key member of our multidisciplinary team, you will provide personalized care and support to patients in their homes, working closely with physicians and other healthcare professionals to ensure the best possible outcomes.

Key Responsibilities
  • Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
  • Conduct thorough assessments, develop and implement individualized care plans, and monitor patient progress.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Educate patients and their families on disease management, medication, and healthy lifestyle habits.
  • Coach patients and their families on safe home practices and provide emotional support as needed.
  • Document patient progress, report changes to physicians, and ensure timely discharge from services.
Requirements
  • Graduate of an accredited Registered Nurse (RN) program.
  • Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license.
  • Minimum of one year of nursing experience, preferably in home healthcare, medical-surgical, or critical care.
