Key Responsibilities- Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
- Conduct thorough assessments, develop and implement individualized care plans, and monitor patient progress.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Educate patients and their families on disease management, medication, and healthy lifestyle habits.
- Coach patients and their families on safe home practices and provide emotional support as needed.
- Document patient progress, report changes to physicians, and ensure timely discharge from services.
- Graduate of an accredited Registered Nurse (RN) program.
- Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license.
- Minimum of one year of nursing experience, preferably in home healthcare, medical-surgical, or critical care.
