RN Care Coordinator

The RN Case Manager serves as a patient advocate, supporting, guiding, and coordinating care for patients, families, and caregivers throughout their health and wellness journeys.
Key responsibilities include developing patient care plans, facilitating efficient throughput, ensuring appropriate resource utilization, and establishing safe discharge plans.
Essential Duties and Responsibilities
Key duties include:
- Screening patients for factors affecting care progression
- Evaluating patient understanding and engagement with care goals
- Arranging services among community agencies, providers, and caregivers
- Providing patients and families with available tools and resources
- Developing clinically appropriate care plans
- Facilitating communication for patient care transitions
- Identifying community resources and advocating for resolution of gaps
- Maintaining compliance with healthcare requirements
- Ensuring appropriate patient status and length of stay
- Addressing avoidable delay practice patterns
- Educating patients on the financial impact of care options
- Tracking and addressing utilization of resources
- Providing clinical information for appeals process
- Participating in performance improvement activities
- Collaborating with utilization review team
- Recognizing and referring quality or risk management situations
Education, Experience, and Qualifications
Requirements include:
- Minimum of Associate Degree in Nursing; BSN preferred
- Minimum 1 year of relevant experience in a medical setting
- Licensed as a Registered Nurse in the state of Florida
- Basic Life Support (BLS) certification required
- Case Management Certification preferred
