Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at The Meadows of Kalida. As a Registered Nurse, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social needs are met.

Key Responsibilities
  • Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
  • Provide direct care to residents, including administering medications, performing wound care, and assisting with daily living activities.
  • Collaborate with the interdisciplinary team to coordinate care and services, ensuring seamless transitions and optimal outcomes.
  • Monitor and report changes in resident condition, identifying potential health risks and taking proactive steps to mitigate them.
  • Communicate effectively with residents, families, and healthcare professionals, ensuring clear and timely information exchange.
Requirements
  • Current RN licensure in the state of Ohio.
  • Minimum of 1 year of experience in long-term care or a related field.
  • Strong clinical skills and knowledge of gerontology.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
