Key Responsibilities

  1. Direct Patient Care
  • Verification of chemotherapy regimens as documented in the electronic medical record.
  • Coordination of chemotherapy and supportive care dispensation.
  • Optimization of medication therapies to minimize and prevent medication errors.
  • Therapeutic drug monitoring for appropriateness of use, dosage, and potential drug interactions.
  • Provision of pharmacokinetic consultations upon request.
  • Patient counseling as applicable.
  • Delivery of medication information to interdisciplinary oncology teams.
  • Development of medication guidelines, order sets, and protocols in collaboration with clinical staff.
  • Identification of opportunities for electronic medical record optimization.
  • Documentation of pharmacotherapy interventions within the electronic medical record.
  1. Education Responsibilities
  • Provision of patient-specific education on chemotherapy regimens as necessary.
  • Facilitation, development, and implementation of educational lectures for pharmacy staff on oncology topics.
  • Delivery of in-service training to medical staff on oncology pharmacology.
  • Delivery of in-service training to nursing staff on oncology pharmacology.
  • Development of guidelines and protocols related to oncology care.
  1. Departmental Responsibilities
  • Ensuring safe, appropriate, and cost-effective medication therapies for patients in accordance with established policies and protocols.
  • Facilitating the development and implementation of treatment guidelines and formulary changes approved by relevant committees, ensuring alignment with national standards.
  • Coordinating the selection and safe use of alternative medications in response to shortages.
  • Providing clinical consultation and clarification to healthcare practitioners.
  • Supporting the clinical initiatives of the pharmacy department.
  • Reviewing the use of targeted medications for compliance with pharmacoeconomic programs.
  • Recommending appropriate and cost-effective therapeutic alternatives to medical staff.
  • Monitoring drug therapy regimens for allergies, contraindications, interactions, and adverse reactions.
  • Assisting in reducing medication costs for oncology patients through careful review of medication regimens.
  • Providing accurate and timely drug information to professional staff.
  • Preparing and reviewing drug use evaluation criteria and adverse drug reaction reports for various committees.
  • Participating in hospital education programs.
  • Coordinating policies and procedures for clinical activities.
  • Engaging in quality improvement activities within the department.
  • Collecting, analyzing, and maintaining data for all documented interventions.
  • Establishing collaborative relationships among the multidisciplinary healthcare team.
  • Maintaining professional competence and promoting development through participation in workshops and professional organizations.
  • Precepting and mentoring pharmacy students from local Colleges of Pharmacy.
  • Training pharmacists and pharmacy students in drug use evaluation.
  • Facilitating discharge and transfer planning related to medication use.
  • Completing special projects as assigned by pharmacy leadership.
  • Participating in Pharmacy and Therapeutics committee activities.

Minimum Qualifications

Required Qualifications:

1. Valid NY State license to practice pharmacy.

2. Two years of post-licensure experience in Oncology Clinical Pharmacy Practice; or

3. Completion of a postgraduate specialty residency or fellowship in Oncology Pharmacy Practice.

Preferred Qualifications:

1. Current Pharmacy Board Certification in Oncology (BCOP).
