About the Role

Pinnacle Services is seeking a skilled Case Manager to join our team in Scott County. As a Case Manager, you will be responsible for providing person-centered services to individuals with intellectual, developmental, or behavioral disabilities.

Key Responsibilities
  • Assess the needs of individuals and develop personalized service plans
  • Coordinate resources and services to meet the needs of individuals
  • Monitor service delivery and work with providers to ensure high-quality care
  • Attend regular meetings with individuals and their interdisciplinary teams
  • Document client-related work and create billable units
Requirements
  • Graduate degree in social work, psychology, sociology, or a related field
  • One year of experience as a social worker, case manager, or care coordinator in a public or private social service agency
  • Valid driver's license and acceptable driving record
  • Vehicle with current insurance
  • Ability to commute to areas of Scott County for meetings
Preferred Qualifications
  • One year of experience in education or treatment of people with developmental disabilities or related conditions
  • Minimum of one course that focuses on developmental disabilities
  • LSW or higher form of BOSW licensure
