Psychiatric Clinical Nurse Specialist

Monadnock Family Services is seeking a highly skilled Psychiatric Clinical Nurse to join our Assertive Community Treatment (ACT) team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and support services to individuals with severe and persistent mental illness.
Key Responsibilities- Provide medication management and administration in conjunction with our psychiatrist.
- Conduct comprehensive psychiatric assessments and develop individualized treatment plans.
- Assess and address physical health needs, making referrals to community physicians as necessary.
- Provide a range of treatments, including psychosocial rehabilitation, to support patient recovery and well-being.
- Collaborate with the ACT team to provide support services and optimize client care.
- Master's, B.A., or associate degree in nursing (LNAs will be considered).
- NH licensed as a registered nurse.
- At least one year of experience in an inpatient or outpatient psychiatric setting (preferred).
- Strong commitment to patient-centered care and the principles of recovery-oriented services.
- Excellent communication and interpersonal skills, with the ability to establish trusting relationships with patients and colleagues.
- Valid driver's license in the state where the team operates.
