Medical Case Manager and Discharge Planner for Acute Hospital Setting

Job Summary
We are seeking an experienced Medical Case Manager and Discharge Planner to join our team at MEDSTAR HEALTH. This is a challenging and rewarding role that requires strong clinical expertise, excellent communication skills, and the ability to work effectively in a fast-paced environment.
About the Role
The successful candidate will be responsible for coordinating patient care from pre-admission through post-discharge, ensuring that patients receive high-quality, cost-effective care that meets their individual needs and outcomes. This will involve collaborating with multidisciplinary healthcare teams, communicating with patients, families, and external stakeholders, and managing a caseload of patients to achieve optimal clinical and resource outcomes.
Main Responsibilities
- Care Coordination: Develop and coordinate plans of care that address the needs of patients across the continuum, including pre-admission, acute hospital stay, and post-discharge phases.
- Patient Communication: Communicate effectively with patients, families, and caregivers to ensure that their needs and preferences are understood and respected.
- Collaboration with Healthcare Teams: Work collaboratively with multidisciplinary healthcare teams to develop and implement plans of care that meet the needs of patients.
- Caseload Management: Manage a caseload of patients, identifying essential resources needed to implement plans of care and achieving optimal clinical and resource outcomes.
- Professional Development: Maintain knowledge of regulatory agencies' requirements, necessary criteria for admission to various care settings, and Medicare's/Medicaid's reimbursement methods for different levels of care.

Qualifications
- Clinical Nursing Experience: 2-3 years of experience as a registered nurse (RN) in a clinical setting.
- Case Management Experience: 1-2 years of experience in case management, preferably in a hospital or healthcare setting.
- Licensure and Certification: Active MD RN license or active compact state RN license, and Basic Life Support for Healthcare Providers certification.
