Acute Care RN Inpatient Case Manager for Discharge Planning

The PRN RN Inpatient Case Manager/Discharge Planner plays a crucial role in ensuring quality and cost-effective patient care outcomes from pre-admission to post-discharge at MEDSTAR HEALTH.
This position involves collaborating with multidisciplinary healthcare teams to develop individualized care plans, assess patient needs, and document progress. A strong focus is placed on managing a patient caseload, adhering to regulatory requirements, and promoting optimal health outcomes within acute hospital settings.
As a member of the Case Management Team, this RN Inpatient Case Manager will apply clinical expertise and medical appropriateness to care coordination and discharge planning. They will facilitate the delivery of quality, cost-effective, patient-centered care from pre-admission through post-discharge.
Key Responsibilities:- Contributes to achieving departmental goals and objectives while adhering to department policies, procedures, quality standards, and safety standards.
- Collaborates with the healthcare team to develop and coordinate patient care plans.
- Communicates effectively with patients, families, healthcare teams, external case managers, community resources, and facilities to address relevant issues and patient/family goals.
- Demonstrates the ability to develop comprehensive care plans addressing patient needs across the continuum, identify specific services, and include family/care-givers in the plan.
- Evaluates and documents patient responses to care plans and makes recommendations for modifications as needed.
- Maintains knowledge of regulatory agencies' requirements, necessary criteria for admission, and Medicare/Medicaid reimbursement methods.
- Manages a caseload of patients, identifies essential resources, and promotes optimal health outcomes.
- Participates in Performance and Service Improvement teams, assisting in program evaluation through customer service surveys, data analysis, and best practice/benchmark data.
- Performs comprehensive assessments to identify patient-specific problems and needs related to diagnosis, treatment, psychosocial, and financial concerns.

- An ADN, BSN, MSN, or CNL degree from an accredited School of Nursing is required.
- 2-3 years of clinical nursing experience, with 1-2 years of case management experience preferred.
