As a Practical Nursing Specialist, you will play a vital role in delivering exceptional patient care and executing skilled nursing tasks within the comfort of patients' homes.
• Deliver services within the defined scope of practice as per state regulations, adhering to the established care plan while collaborating with the healthcare team.
• Maintain the integrity of the care plan without modifications.
• Document a progress note for each visit and ensure timely submission of these notes to the agency.
• Continuously assess and communicate the nursing needs of patients, providing guidance to their families on care and maintaining a healthy living environment.
• Observe, document, and report to the supervising nurse or physician regarding the patient's response to treatment and any changes in their condition.
• Valid LPN licensure in the state of practice.
• Current Driver's License, vehicle insurance, and reliable transportation.
• CPR Certification is mandatory.
