Full-time Registered Nurse in Home Health Care Specialist

The RN Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence.
Key Responsibilities- Practice is within the scope defined by the Washington State Nurse Discipline Practice Act, based on the nursing process model and concept of community health nursing therapy.
- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Conduct regular assessments and evaluations to ensure the patient's needs are being met and make adjustments to the care plan as needed.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited school of nursing
- Bachelor's Degree
- ~2 years full-time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility
- ~1 year recent, hands-on technical clinical experience (occurring within the last three years) in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management
- ~Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes
- ~Rehab nursing experience
- ~Experience with office software programs including Outlook, Word, and/or Excel
