- Work alongside healthcare professionals to create and implement individualized medication plans.
- Continuously monitor medication therapies to achieve optimal therapeutic outcomes.
- Adjust medication regimens based on thorough data analysis and patient assessments.
- Follow established protocols for collaborative pharmacokinetic drug therapy management.
- Engage with patients and their families to gather vital information regarding medication history and allergies.
- Document all relevant information accurately in patient medical records.
- Provide guidance to patients and families on medication usage, storage, and adherence.
- Educate patients on the proper use of medication-related devices.
- Offer pharmacokinetic dosing recommendations to healthcare providers.
- Manage medication orders, including dosage adjustments and laboratory test requests.
- Ensure the effectiveness and accuracy of the medication distribution system.
- Review medication orders for compliance with formulary guidelines.
- Clarify and intervene on medication orders as necessary.
- Verify the accuracy of medication preparation and dispensing by pharmacy staff.
- Supply medical personnel with essential medication information.
- Identify and address potential medication safety concerns proactively.
- Analyze patient records for adverse drug events and assist in developing treatment strategies.
- Evaluate adverse drug events for severity and preventability.
- Confirm proper medication delivery and administration.
- Maintain secure and appropriate medication storage.
- Participate in clinical trials as required.
- Engage in problem-solving and consult with supervisors when needed.
- Foster a collaborative environment within the Labor-Management Partnership.
- Report any discrepancies to the appropriate medical or administrative personnel.
- Develop and test specifications for protocols and pharmaceutical care applications.
- Participate in ongoing education and training initiatives.
- Create and deliver educational programs for pharmacy staff, medical personnel, and patients.
- Actively contribute to rounds, committees, and quality management programs.
- Engage as a member of the interdisciplinary team during medical emergencies.
