We're looking for a skilled Registered Nurse (RN) Case Manager to join our team. In this role, you'll be the point of contact for all disciplines involved in providing care to patients and oversee the frequency of visits for the episode.

As a RN Case Manager, you'll consult with physicians and office staff to ensure patient care is delivered effectively. You'll also perform accurate OASIS collection, ensure medication profiles are up-to-date, and report lab values to physicians in a timely manner.

Responsibilities:

As a RN Case Manager, your key responsibilities will include:

  • Performing visits and coordinating client care
  • Overseeing the frequency of visits for the episode
  • Consulting with physicians and office staff
  • Performing accurate OASIS collection
  • Ensuring medication profiles are up-to-date
  • Reporting lab values to physicians in a timely manner

Qualifications:

To be successful in this role, you'll need:

  • Current licensure as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience
  • Demonstrated knowledge and skill in current nursing practice
  • A valid state driver's license and automobile liability insurance
