About the Role

Curana Health, a leading provider of value-based primary care services, is seeking a Certified Wound Care Nurse Practitioner to join our team. This role will involve delivering high-quality wound care, chronic medical conditions, and comprehensive medical care within the Post-Acute Care setting.

Responsibilities

  • Performing physical exams, wound care assessments, and patient observations to identify areas for improvement
  • Creating and implementing patient care plans, analyzing diagnostic test results, and diagnosing health issues
  • Ordering and administering diagnostic tests, medications, and treatments as needed
  • Collaborating with healthcare professionals and families to ensure comprehensive care
  • Monitoring patient progress and modifying treatment plans as necessary
  • Performing wound care treatments and other duties as assigned

Requirements

  • Valid nurse practitioner license in the state of operations and current board certification in the field
  • Masters Degree in Nursing and Wound Care Certification
  • Strong leadership, interpersonal, and problem-solving skills
  • Excellent written and oral communication skills
  • Punctuality, organizational skills, and ability to function within a team
