Your efforts will help alleviate fears, instill hope, and enable patients to appreciate the beauty in each moment, while ensuring they receive the respect and dignity they deserve.
Key Responsibilities:
• Support patients with personal care routines, including bathing, oral hygiene, skincare, hair grooming, and shaving.
• Aid patients with activities of daily living (ADLs) such as toileting, dressing, and feeding.
• Assist with mobility and transfer tasks.
• Facilitate range of motion exercises as prescribed.
• Position patients for optimal comfort.
• Help with the use of medical equipment and assistive devices.
• Address nutritional and dietary needs as directed by the Nurse Case Manager.
• Maintain a safe and hygienic environment for patients by changing bed linens as needed, tidying living spaces, and adjusting lighting and object placement for safety and comfort.
• Report any notable changes in the patient's condition or caregiver dynamics to the Nurse Case Manager.• Document all care activities in accordance with agency standards.
• Collaborate continuously with the Interdisciplinary Team to ensure quality and continuity of care.
• Adhere to all established agency policies and procedures.
• Follow all health and safety regulations and requirements.
Knowledge and Skills Required:
• Ability to remain adaptable and organized in planning.
• Capacity to foster and maintain effective working relationships with staff, clients, family members, the public, and representatives from other organizations.
• Proficient communication skills across all levels of interaction.Experience and Educational Requirements:
• Current CNA Certification
• Minimum of one year of direct patient care experience
