About the Role

The Social Services Director is a key member of the Parkview Healthcare & Rehab Center team, responsible for providing exceptional support to residents and their families. Under the guidance of the Administrator, this role oversees the admissions process and serves as a valuable resource for counseling, family dynamics, and interactions.

Key Responsibilities
  • Admissions Process Management: Manage the admissions process from hospital, home, or other prior living arrangements, ensuring a seamless transition for residents.
  • Marketing and Outreach: Promote the facility to referral sources, fostering strong relationships and increasing visibility.
  • Payer Source Coordination: Effectively communicate with payor sources, including Medicare, Medicaid, and insurance providers, to ensure timely and accurate reimbursement.
  • Psychosocial History and Treatment Planning: Conduct thorough psychosocial histories to identify pre-injury issues, and establish individualized treatment plans as needed.
  • Counseling and Support: Provide counseling to clients, families, and significant others, offering emotional support and guidance throughout their journey.
  • Team Collaboration: Participate in team rounds and family conferences, ensuring a unified approach to resident care.
  • Record Keeping and Reporting: Maintain accurate records and prepare reports according to facility procedures, facilitating data-driven decision making.
  • Professional Development: Engage in internal and external professional educational activities to enhance personal and professional growth.
  • Public Relations: Perform public relations tasks as directed by the supervisor, promoting a positive image of the facility.
  • Resident Scheduling: Coordinate resident schedules with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist, and other disciplines as needed.
  • Discharge Planning: Assist in patient discharge planning, ensuring a smooth transition to the next level of care.
  • Post-Discharge Follow-Up: Conduct post-discharge follow-up as needed, maintaining a connection with residents and their families.
Requirements
  • Education and Certification: Must hold a Social Services Designee Certification or be able to obtain within 90 days of hire.
  • Experience: Minimum of two years of experience in a healthcare setting, preferably in a Skilled Nursing Facility (SNF).
