This role is integral to our mission of enhancing the lives of our residents by creating new opportunities for engagement and support.

Key Responsibilities:

Role Summary:

The Senior Community Service Facilitator is responsible for orchestrating programs and services that empower residents to maintain a high quality of life while aging in place. This includes:

  • Conducting assessments to identify resident and family needs, as well as external resources to address those needs.
  • Networking with local organizations to discover affordable and innovative services for residents.
  • Performing psychosocial evaluations through interviews with residents, families, and relevant parties to assess current conditions and potential adjustments.
  • Providing comprehensive case management and referral services, ensuring that resident files are meticulously maintained and updated.
  • Coordinating services that promote optimal living conditions, focusing on wellness and aging in place philosophies.
  • Developing and executing Care Plans and conducting Quarterly Monitoring for residents requiring referrals, with diligent follow-up on all services.
  • Preparing Semi-Annual Performance Reports to secure funding for the Service Coordination program.
  • Engaging residents to identify their needs and facilitating necessary referrals, while promoting participation in both on-site and off-site programs.
  • Assisting in the arrangement of home care services to support independent living and aging in place, including the creation of a Resource Directory listing service providers.
  • Providing ongoing education to residents and staff regarding service availability, application processes, resident rights, health, safety, fitness, and aging-related issues.
  • Coordinating counseling services, occasional nursing visits, preventative health screenings, and legal advocacy.

Required Skills and Abilities:
Excellent communication, comprehension, and interpersonal skills are essential. Proficiency in Windows, Microsoft Office Suite, and resident management software systems is preferred.

Qualifications:

Educational Background:
A minimum degree in social work, psychology, or gerontology is preferred.

Professional Experience:
At least two years of experience in social service delivery, particularly with elderly populations or families. A demonstrated understanding of supportive services and resources available in the community is crucial. The ability to advocate, organize, problem-solve, and deliver results for residents is essential.
