Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients' physical and emotional needs, develop individualized care plans, and implement interventions to achieve optimal patient outcomes.
- Collaboration and Communication: Work effectively with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to coordinate patient care and ensure seamless transitions.
- Patient Education and Support: Provide patients and their families with education and support to promote health literacy, self-management, and empowerment.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care, safety, and satisfaction.
- Leadership and Professional Development: Demonstrate leadership skills, take initiative, and engage in ongoing professional development to stay current with best practices and evidence-based care.
- Education: Completion of an accredited or approved program in Nursing.
- Licensure and Certification: Registered Nurse license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
- Experience: Typically requires 1 year of experience in clinical nursing.
- Skills and Abilities: Demonstrated ability to work effectively in a fast-paced environment, excellent communication and interpersonal skills, and proficiency in electronic medical records systems.
- Knowledge and Competencies: Strong understanding of nursing principles, patient safety, and quality improvement initiatives.
