  • Formulate and implement age-appropriate, culturally sensitive care plans.
  • Coordinate patient care across multiple disciplines and caregivers.
  • Provide leadership and support to team members, promoting a culture of excellence and teamwork.
  • Assign, delegate, and supervise other employees, ensuring continuity of care and established outcomes.
  • Educate patients about healthy practices and treatment modalities.
  • Participate in process improvement activities, focusing on increasing continuity and minimizing rework.
Requirements
  • RN licensure issued by the PA Board of Nursing.
  • Bachelor of Science in Nursing (BSN) or higher-level nursing degree.
  • CPR Certification - American Heart Association (AHA) Basic Life Support.
  • A healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs.
