Long Term Care Coordinator I

Sunflower Health Plan is seeking a highly skilled Long Term Care Coordinator I to join our team. As a key member of our care management team, you will play a critical role in developing and implementing holistic care plans that enable quality, cost-effective healthcare outcomes for our members.
Key Responsibilities- Evaluate Member Needs: Assess the needs of our members, identify resources available, and recommend and/or facilitate plans for the best possible outcomes.
- Develop Care Plans: Assist in developing ongoing long-term care plans and work to identify providers, specialists, and community resources needed for long-term care.
- Coordinate Care: Coordinate between members, families/caregivers, and care provider teams to ensure identified services are accessible to members.
- Provide Resource Support: Offer resource support to members and their families/caregivers for various needs, such as employment, housing, participant direction, independent living, justice, foster care, and more.
- Monitor Care Plans: Monitor care plans, member status, and outcomes, and provide recommendations to care plans based on identified member needs.
- Interact with Healthcare Providers: Interact with long-term care healthcare providers and partners as needed to ensure member needs are met.
- Collect and Maintain Member Information: Collect, document, and maintain long-term care member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators.
- Perform Site Visits: Perform home and/or other site visits to assess member needs and collaborate with healthcare providers and partners.
- Provide Education and Feedback: Provide and/or facilitate education to long-term care members and their families/caregivers on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits. Provide feedback to leadership on opportunities to improve and enhance quality of care and service delivery for long-term care members in a cost-effective manner.
We are seeking a candidate with a Bachelor's degree and 2-4 years of related experience or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope, and skill reflective of the level of this position.
