Long-Term Services Support Coordinator - Clinician
Job Overview:
The Long-Term Services Support Clinician operates under the guidance of a Registered Nurse (RN) and holds comprehensive responsibility for the member's case management, in accordance with state regulations and contractual obligations. This role plays a vital part in the care coordination process by executing tasks within the licensed scope, such as assisting the RN with both telephonic and in-person evaluations to identify, assess, coordinate, and manage the needs of members, encompassing physical health, behavioral health, social services, and long-term support services.
Key Responsibilities:
- Support the RN in pinpointing members at risk for complications.
- Gather clinical information as instructed by the RN. Aid the RN in recognizing members who may benefit from alternative care levels or waiver programs.
- Deliver all collected data to the RN, who will verify, interpret, and conduct further assessments as needed, ultimately developing, monitoring, and adjusting the member's care plan to address their needs.
- Engage in the coordination of care for members with chronic conditions, co-morbidities, and/or disabilities as directed by the RN, collaborating with the RN, member, and healthcare team to ensure efficient and cost-effective use of health benefits.
- Utilize decision-making skills based on the member's current needs, requiring an understanding of disease processes and terminology, as well as the application of clinical guidelines, without necessitating nursing judgment.
- Must possess an LPN/LVN, LSW, LCSW, LMSW, or a license other than RN as per state law, along with a Nursing Diploma or Associate's Degree in Nursing or a related field, and at least 2 years of experience working with individuals facing chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar capacity; or any combination of education and experience that provides an equivalent background.
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other applicable state license required.
- Familiarity with Microsoft Office Suite, including Outlook, Word, and Excel is preferred.
- Prior experience in case management is advantageous.
- Willingness to travel to various work sites as necessary.
