About the Role

PACE of Southwest Michigan is seeking a highly skilled Care Coordinator to join our Social Work team. As a key member of our interdisciplinary team, you will play a vital role in coordinating and overseeing the delivery of healthcare services to our elderly participants.

Key Responsibilities
  • Participate in initial assessments, care planning, and periodic re-assessments (every 6 months) of the participant's case.
  • Communicate participant updates to the Interdisciplinary Team and appropriate parties.
  • Document updates in the participant's medical record, including assessments, and create and maintain behavior modification plans.
  • Coordinate admissions and discharges from facilities deemed appropriate by the IDT and manage the dis-enrollment process from PACE of Southwest Michigan.
  • Refer participants and families to appropriate community agencies or facilities and act as a liaison with various organizations and as an advocate for participants.
  • Participate in program and policy development and actively participate in the Quality Improvement Plan process.
  • Facilitate the Medicaid application and redetermination process with the Michigan Department of Health and Human Services (MDHHS) and ensure timely completion.
  • Serve on internal and external committees when appropriate.
  • Complete home visits as needed.
  • Abide by all established PACE of Southwest Michigan policies, rules, and regulations.
Requirements
  • Master's degree in Social Work.
  • Two years' experience in a health-related industry is preferred.
  • Licensed MSW in the State of Michigan, valid Driver's License with a vehicle and safe driving record.
  • CPR certification upon hire.
