The Clinical Quality Outcomes Coordinator plays a vital role in enhancing patient care by coordinating services and improving communication within our healthcare team. You will work closely with providers, nurses, and other staff to ensure seamless transitions and optimal patient outcomes.
Primary Responsibilities- Actively participate in patient care planning and execution to achieve clinical goals set by supervisors.
- Coordinate patient care through pre-visit planning, one-on-one communication, and documentation of services accessed or health concerns.
- Utilize managed care portals to access ER/Hospitalization reports and identify potential gaps in care.
- Collaborate with the care team to proactively address identified care gaps during patient visits.
- Assist the healthcare team in monitoring patient health outcomes and identifying areas for improvement.
- Advocate for individual patient needs by communicating opportunities for interventions and addressing gaps in care reported by managed care companies and quarterly assessments.
- Follow up with patients after hospitalizations, emergency visits, or institutional admissions to ensure smooth transitions back to primary care.
- Build strong relationships with referral sources, community resources, and other healthcare providers.
- Contribute to data collection efforts related to service delivery and patient outcomes.
- Participate in team meetings and contribute to a collaborative work environment.
