The Community Health Navigator is a vital role at LifeBridge Health, where we strive to CARE BRAVELY for over 1 million patients annually. As a member of our team, you will be responsible for providing non-clinical care coordination, health coaching, and/or navigation support to clients.
Key Responsibilities- Assess client's health and social needs and work with clients to develop and implement individualized plans to support clients with meeting their health goals.
- Provide health education and coaching to achieve improved client-level health outcomes.
- Assist clients with accessing primary care, managing their chronic disease, and obtaining access to other community resources.
- Address barriers to care and provide support to clients in recovery from, or at risk of developing alcohol and/or substance use dependence or other chronic diseases.
- Entry-level knowledge and basic employment skills.
- High school diploma or GED equivalent or high school certificate.
- 1-3 years of experience in a related field.
- Maryland Peer Recovery Specialist Certification (CPRS).
