Job Summary

  • Identify patients at risk for adverse outcomes during transition from one level of care to another

Requirements

To be considered for this role, candidates must:

  • Hold current licensure as a Registered Nurse in the state of practice
  • Possess certification in Case Management (preferred)
  • Have a minimum of 2 years of experience in case management or clinical nursing
