Key Responsibilities:- Facilitates the assessment of patient conditions utilizing the Minimum Data Set, addressing biophysical, psychosocial, and age-specific requirements.
- Participates in interdisciplinary meetings to gather insights, communicate updates, and maintain accurate records.
- Collaborates with healthcare professionals to create a tailored interdisciplinary care plan.
- Organizes care plan discussions with the interdisciplinary team, residents, and their families.
- Assesses patient status, nursing directives, and the effectiveness of the care plan.
- Implements a patient education strategy based on individual needs, as directed by medical professionals, including guidance for patients and families.
- Engages in educational initiatives for residents and their families.
- Manages the electronic submission of necessary documentation to regulatory bodies in accordance with company policies and legal requirements.
- Keeps a systematic record of MDS schedules.
- Ensures accurate and timely data entry of resident information into relevant computerized MDS systems.
- Continuously enhances knowledge related to data management and technology.
- Verifies that all supporting documentation is complete and precise.
- Records patient status following unit-specific protocols.
- Exhibits clinical proficiency in evaluating patient/resident needs and implementing suitable solutions in line with regulatory standards.
- Contributes to performance improvement initiatives within the unit.
- Acts as a substitute for the Admission Coordinator when necessary to ensure ongoing assessments and screenings for admissions.
Additional Information
Educational Qualifications:
- Requires an Associate's (A.A.) or Bachelor's degree (BSN) from an accredited nursing program.
Licensure/Certification:
- BCLS, NC RN License required.
Professional Experience:
- Minimum of two years in long-term care along with one year of MDS experience. A solid understanding of managed care and Medicare/Medicaid payment systems is essential.
Skills and Abilities:
Language Proficiency:
Ability to read, analyze, and interpret clinical data and technical procedures. Proficient in documenting clinical information and effectively communicating with patients, families, and healthcare staff.
Mathematical Proficiency:
Ability to apply mathematical concepts such as fractions, percentages, and ratios to real-world scenarios.
Analytical Skills:
Ability to identify problems, gather data, establish facts, and draw valid conclusions. Capable of interpreting a wide range of technical instructions.
