We understand that navigating end-of-life challenges can be a profound experience for all parties involved. This is why Dignity Health Hospice is dedicated to delivering compassionate and supportive care to our patients and their families.
Hospice care is characterized by a collaborative approach that encompasses expert medical treatment, symptom and pain management, as well as emotional and spiritual support tailored specifically to the needs of the patient.
Our primary objective is to honor our patients' wishes while providing support to their families and caregivers. Hospice services aim to help patients maintain the highest possible quality of life. We are committed to caring for the entire individual, not merely the illness. Additionally, Dignity Health Hospice offers bereavement services for a full year following a loss, with a trained bereavement team available to assist families and caregivers in coping with their grief.
Responsibilities
In alignment with the Marian Medical philosophy, the Clinical Pharmacist is tasked with the procurement, manufacturing, compounding, dispensing, distribution, storage, and administration of medications, biologicals, and chemicals in accordance with established professional standards, state and federal regulations, as well as departmental policies and procedures.
This role includes providing drug information to healthcare professionals, patients, their families, and the general public. The Clinical Pharmacist must demonstrate sound professional judgment in executing job functions and possess effective communication skills, both verbal and written. Attention to detail is crucial, along with the ability to analyze complex tasks and systems, designing and implementing the most effective solutions.
Furthermore, the Clinical Pharmacist should be adept at working collaboratively with team members, patients, their families, and the public. A comprehensive understanding of the practices, principles, methods, and equipment used in pharmaceutical services is essential. The ability to organize and prioritize tasks effectively is also required. A current and valid California pharmacy license is mandatory.
Qualifications
Minimum Qualifications:
- One (1) year of experience in acute care hospital pharmacy or home infusion therapy.
- Current and valid California pharmacy license.
Preferred Qualifications:
- Pharm.D. degree preferred.
- Experience in I.V. admixture, unit dose distribution, pharmacokinetics dosing, pharmacy intervention, and medication usage evaluation.
