We are seeking a compassionate and skilled Home Health Aide to join our Hospice team at Horizon Home Care & Hospice. As a Home Health Aide, you will play a vital role in providing high-quality care to patients in their homes, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Key Responsibilities- Provide direct assistance with activities of daily living, including bathing, dressing, and toileting, under the supervision of a Registered Nurse or Therapist.
- Support patient care by reviewing care plans and notifying Case Managers of any issues or concerns.
- Explain care being provided to patients and involve them in their care.
- Complete mobile care upon arrival and departure from patients' homes, including tasks outlined in the care plan.
- Work cooperatively with patients, care providers, and family members to ensure seamless care delivery.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. You will also need to be able to maintain patient confidentiality and adhere to Agency policies and procedures.
