Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Mercy Home Health team. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.

Responsibilities
  • Assess, plan, implement, document, evaluate, and teach patients' needs and care
  • Provide skilled nursing services to patients in their homes under the direction of a physician
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Manage a caseload of patients with diverse health conditions, providing education, treatment, and emotional support
  • Document patient progress, medication, and plan of care in a concise and accurate manner
Requirements
  • 1 year of experience required; Home Health experience preferred
  • Graduation from an accredited nursing school
  • Current RN license in the state of Missouri
  • BLS CPR Certification through the American Heart Association
  • Ability to anticipate and respond to changes in clinical status of patients
