The Lighthouse Community Hospice Inc is seeking a compassionate and skilled Hospice Care Assistant to join our team. As a Hospice Care Assistant, you will play a vital role in providing high-quality care and support to patients and their families in their homes.
Key Responsibilities- Provide personal care services to patients, including bathing, dressing, and grooming, as assigned.
- Assist patients with daily living activities, such as feeding, toileting, and ambulation.
- Monitor patients' conditions and report any changes or concerns to the Case Manager.
- Assist with light housekeeping and laundry duties to maintain a clean and safe living environment.
- Provide emotional support and companionship to patients and their families.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Document patient care activities and progress in accordance with organizational policies and procedures.
- Meet the training requirements of the State, including completion of a minimum of 75 hours of classroom and supervised practical training.
- Be at least 18 years of age.
- Have the ability to read and follow written instructions and document care given.
- Understand hospice philosophy and be comfortable providing specialized care to the terminally ill.
- Have satisfactory references from previous employers.
- Be self-directing with the ability to work with little direct supervision.
- Have empathy for the needs of the ill, injured, frail, and the impaired.
- Possess and maintain current CPR certification.
- Demonstrate tact, patience, and good personal hygiene.
- Be a licensed driver with an automobile that is insured in accordance with organization requirements and is in good working order.
- Work in a community home environment.
- May be required to travel in and out of an automobile.
