Job Summary

We are seeking a skilled Occupational Therapist to join our team at Discovery At Home. As a key member of our home health team, you will provide high-quality occupational therapy services to clients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities
  • Evaluate clients' functional abilities and develop comprehensive treatment plans to improve independence and physical and mental functioning.
  • Conduct initial and ongoing assessments of clients' needs, including OASIS assessments, and develop and implement individualized care plans.
  • Collaborate with clients, family members, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide education and training to clients and their caregivers on therapeutic self-care and creative activities to promote independence and well-being.
  • Supervise and counsel clients and their families on occupational therapy techniques and strategies to enhance daily living skills and overall quality of life.
  • Participate in quality assessment and performance improvement activities to ensure high-quality care and continuous improvement.
Requirements
  • Master's degree in Occupational Therapy from an accredited program.
  • Current licensure or registration as an Occupational Therapist in the state of practice.
  • At least one year of experience in home health or institutional settings, with a strong background in occupational therapy.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
