Job Summary

Discovery at Home is seeking a skilled Occupational Therapist to join our team. As a key member of our interdisciplinary team, you will provide high-quality occupational therapy services to clients in their homes, promoting independence and improving their overall quality of life.

Responsibilities
  • Evaluate clients' needs and develop personalized care plans to address their physical, emotional, and cognitive abilities.
  • Conduct comprehensive assessments, including OASIS evaluations, to identify areas for improvement and develop strategies for enhancing clients' independence.
  • Collaborate with clients, family members, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide education and training to clients and their caregivers on proper use of wheelchairs, braces, splints, and other assistive devices.
  • Supervise and mentor Home Health Aides and Occupational Therapy Assistants to ensure high-quality care delivery.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in occupational therapy.
Requirements
  • Master's degree in Occupational Therapy from an accredited program.
  • Current licensure or registration as an Occupational Therapist in the state of practice.
  • At least one year of experience in home health or institutional settings, with a focus on occupational therapy.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to travel to clients' homes and work in a fast-paced, dynamic environment.
