Job Summary

The RN Care Manager is a key member of our interdisciplinary team, responsible for coordinating patient care and services. This role requires a collaborative approach, working with patients, families, care teams, payers, and external entities to ensure a safe transition from our facility to the next provider or care setting.

Responsibilities
  • Coordinate patient care and services, promoting quality care through a collaborative process
  • Work with patients and families to understand their needs and preferences
  • Collaborate with care teams, payers, and external entities to ensure seamless transitions
  • Develop and implement individualized care plans, taking into account patient goals and preferences
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
  • Communicate effectively with patients, families, and care teams to ensure understanding and cooperation
  • Participate in interdisciplinary team meetings to discuss patient care and services
  • Stay up-to-date with relevant laws, regulations, and industry standards, ensuring compliance and best practices
Requirements
  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Current licensure/registration in applicable State
  • One (1) year of clinical experience in post acute care setting preferred
  • Prior case management, utilization review, and discharge planning experience preferred
  • Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
Essential Functions
  • Demonstrate efficient use of relevant computer systems
  • Serve as liaison to external case managers, family, physicians, and community resources
  • Train and educate patients, families, associates, and other providers of care
  • Implement the standards of practice for care management, ethical performance, and functions relevant to coordination of care
  • Exhibit excellent customer service and a positive attitude towards patients
  • Assist in the evacuation of patients
  • Demonstrate dependable, regular attendance
  • Concentrate and use reasoning skills and good judgment
  • Communicate and function productively on an interdisciplinary team
