The RN Care Manager is a key member of our interdisciplinary team, responsible for coordinating patient care and services. Responsibilities- Coordinate patient care and services, promoting quality care through a collaborative process
- Work with patients and families to understand their needs and preferences
- Collaborate with care teams, payers, and external entities to ensure seamless transitions
- Develop and implement individualized care plans, taking into account patient goals and preferences
- Monitor and evaluate patient progress, making adjustments to care plans as needed
- Communicate effectively with patients, families, and care teams to ensure understanding and cooperation
- Participate in interdisciplinary team meetings to discuss patient care and services
- Stay up-to-date with relevant laws, regulations, and industry standards, ensuring compliance and best practices
- Nursing diploma (associate's or bachelor's degree in nursing)
- Current licensure/registration in applicable State
- One (1) year of clinical experience in post acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
- Demonstrate efficient use of relevant computer systems
- Serve as liaison to external case managers, family, physicians, and community resources
- Train and educate patients, families, associates, and other providers of care
- Implement the standards of practice for care management, ethical performance, and functions relevant to coordination of care
- Exhibit excellent customer service and a positive attitude towards patients
- Assist in the evacuation of patients
- Demonstrate dependable, regular attendance
- Concentrate and use reasoning skills and good judgment
- Communicate and function productively on an interdisciplinary team
The RN Care Manager will work in a fast-paced, dynamic environment, requiring strong organizational and communication skills. The ability to multitask, prioritize, and manage time effectively is essential. The successful candidate will be a team player, committed to delivering high-quality patient care and services.
