Role Summary
The Licensed Practical Nurse (LPN) will operate under the general supervision of a Registered Nurse (RN) or Physician (MD/DO) within the Community Living Center (CLC). The CLC is dedicated to providing resident-centered care for Veterans of various age groups.
The LPN will be responsible for delivering age-appropriate nursing care to adult and geriatric patients with diverse physical, mental, behavioral, or psychosocial needs.
Responsibilities Include:
- Participating in interdisciplinary team meetings and contributing to the Nursing Care Plan.
- Assisting residents with daily living activities, including bathing, feeding, and mobility.
- Educating patients and families about common health issues and treatment plans.
- Ensuring a safe and therapeutic environment for patients.
- Performing various nursing procedures, including wound care and medication administration.
- Monitoring and documenting patient responses to treatments and medications.
- Collaborating with healthcare teams to promote optimal patient care.

Various shifts based on patient care needs; details to be discussed during the interview.

Qualifications:
