Job Summary:

Total Care Connections is seeking a compassionate and organized Care Coordinator to join our Phoenix team. As a key member of our care team, you will play a crucial role in supporting our Care Manager by expanding billable hours, ensuring regulatory compliance, and delivering exceptional care to seniors and disabled adults.

Key Responsibilities:
  • Conduct client assessments and reassessments, gathering relevant information and documenting assessment data.
  • Develop comprehensive care plans addressing all pertinent areas of the assessment, incorporating private and community resources.
  • Assist in client care coordination with outside resources, making and attending appointments as needed.
  • Ensure timely updates to care plans based on reassessments or evolving client needs.
  • Communicate all care plan modifications to caregivers and the operations team promptly.
  • Conduct new caregiver orientations and ongoing training, ensuring caregivers are trained to company standards.
  • Facilitate DCW/PCW training and oversee skills evaluations.
  • Ensure the branch office adheres to local licensure laws and Medicaid regulations.
Requirements:
  • At least one year of care supervision, coordination, or management experience.
  • Ability to become DCW Trainer certified.
  • Excellent customer service skills.
  • Skilled in caregiving, with expertise in advanced transfers and hands-on care.
  • Proficiency with computers and technology.
  • Strong organizational skills and attention to detail.
  • Team player with workplace flexibility.
  • Ability to function well in a high-paced and sometimes stressful environment.
  • Reliable vehicle and clean driving record.
