Registered Nurse Case Management Specialist

Position Summary
In this role, you will operate under general supervision, utilizing Case Management Guiding Principles to collaborate with patients and healthcare providers, ensuring access to services and a seamless transition throughout the continuum of care.
Your responsibilities will include working alongside healthcare professionals, community resources, liaisons, and families to facilitate coordinated admission and discharge planning.
You will advocate for and support the case management process, employing the Physician-Centric Care Management Model as a framework.At our organization, caregivers are not merely valued; they are essential.

We are currently seeking candidates for the Case Management department.
Case Manager RN - Part-time, 0.6 FTE, Day shift - These positions are designated for weekend coverage (Friday, Saturday, Sunday - 8-hour shifts; and Saturday, Sunday, Monday - 8-hour shifts; totaling 24 hours per week).
Case Manager RN - Per diem, 0.01 FTE, Day shift - Hours will be as needed and are not guaranteed.
Qualifications
Required:
- Nursing degree/diploma
- Alaska Registered Nurse License upon hire
- 2 years of clinical nursing experience
Preferred:
- Bachelor's Degree in Nursing or
All Registered Nurses without a minimum of a Bachelor's degree in Nursing must obtain a Bachelor's degree or higher within 3 years of hire.
Any previously agreed-upon effective date for obtaining a Bachelor's degree in nursing between the individual and facility will remain in effect.
Any collective bargaining contract with specific time frames for obtaining a Bachelor's degree in nursing will supersede the above point and will remain in effect.
Exceptions will be considered on a case-by-case basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader.
RNs employed prior to a specified date are encouraged to obtain a Bachelor's degree or higher in Nursing, but this will not be a condition of employment.- 3 years of clinical nursing experience
- 1 year of utilization management-review and discharge-planning experience
