Role Summary and Qualifications
The Case Management Specialist (CMS) ensures the delivery of high-quality, patient-centered care by overseeing rehabilitative services to achieve optimal outcomes. The CMS provides, coordinates, and directs care tailored to the specific needs of each rehabilitation patient. The CMS collaborates with the Rehabilitation Program Director and Facility Case Management Director regarding departmental operations. The CMS coordinates efforts within the Rehabilitation team.
Key Responsibilities:
- Facilitate program orientation for patients, families, and caregivers, covering the case manager's role, rehabilitation philosophy, and criteria for continued stay and discharge. This will also encompass Medicare and insurance benefits, grievance procedures, treatment planning processes, and rights and responsibilities.
- Act as the coordinator for patient, family, and caregiver education, promoting their involvement in team discussions related to plans, goals, and status through Family Conferences and other interactions.
- Ensure the execution of the patient's treatment plan that aligns with their strengths, needs, and preferences, facilitating their involvement throughout the rehabilitation journey.
- Document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating the social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Take responsibility for fostering consistent, positive patient interactions that enhance the agenda of exceptional patient service.
Qualifications:
- Current licensure as a Registered Nurse in the State of Alaska or an RN compact license from participating compact states.
- Associate Degree in Nursing required; BSN preferred.
- 2+ years of experience in case management or 3+ years of clinical nursing experience required.
- American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification.
- Certification in case management or utilization review preferred.
- InterQual experience preferred.
