  • Service Delivery: Guarantees timely provision of rehabilitation services to participants in need.
  • Regulatory Compliance: Ensures adherence to all PACE regulatory requirements related to the Rehabilitation department.
  • Compliance Assistance: Aids in maintaining CMS Regulatory Compliance, including the management of SDR, grievances, and regulatory assessments.
  • Caseload Management: Supports the management of active caseloads for rehabilitation patients to ensure adequate coverage.
  • Team Collaboration: Regularly participates in PACE Interdisciplinary Team meetings and facilitates discussions as necessary.
  • Operational Oversight: Manages all operations related to PACE Therapists and Rehabilitation Assistants, including equipment management.
  • Standards Establishment: Develops and implements minimum care standards for therapists.
  • Program Development: Assists in the initiation of new PACE programs and oversees rehabilitation operations at various locations, which may involve travel to different sites.
  • Orientation and Procedures: Creates comprehensive orientation guides and standard operating procedures for the PACE Rehabilitation program.
Qualifications, Skills, and Abilities:
  • Education: Bachelor's degree in Physical Therapy or Occupational Therapy required; Master's or Doctorate preferred.
  • Licensure: Current valid Florida PT or OT license.
  • Experience: Minimum of 5 years of full-time experience as a PT or OT preferred; at least 2 years in a supervisory role required.
  • Population Experience: At least 1 year of experience working with frail or elderly populations.
  • Travel Requirements: Ability to travel within designated counties.
  • Certifications: Current CPR certification required.
  • Background Check: Florida AHCA Level 2 background check clearance is necessary.
