About the Role:

At Trinity Health, we are seeking a highly skilled Physical Therapist to join our team in the Morrisville/Yardley/Levittown area.

The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering high-quality patient care.

Job Summary:

  • We are looking for a licensed Physical Therapist to consult, evaluate, plan, and administer skilled physical therapy services prescribed by a physician to restore function, relieve pain, and prevent disability following illness, disease, injury, or surgery.
  • Collaborate with inter-disciplinary team members to ensure patient needs are met, and quality of care is achieved.
  • Act as the patient care manager, managing, overseeing, and providing primary patient care delivery to a select group of patients, supporting the patient-centered care model, ensuring quality, and maintaining open communication.

Key Responsibilities:

  • Evaluate patients' conditions and develop personalized treatment plans to improve their functional abilities.
  • Administer physical therapy interventions, such as exercises, manual therapy, and education, to promote recovery and prevent further injury.
  • Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to coordinate patient care and achieve optimal outcomes.
  • Communicate effectively with patients, families, and caregivers to educate them about treatment plans, progress, and discharge instructions.

Requirements:

  • Graduate of an approved PT educational program.
  • Bachelor's, Master's, or Doctorate degree preferred.
  • Licensure as a PT in the state of PA.
  • One year of experience as a professional therapist, with home care experience a plus.
  • Current driver's license and reliable transportation.
