Job Title: Private Duty Nurse LPN

We are seeking a skilled and compassionate Private Duty Nurse LPN to join our team at Aveanna. As a Private Duty Nurse LPN, you will be responsible for delivering high-quality patient care in a private setting, working closely with our clients and their families to provide personalized care and support.

Job Summary:

The Private Duty Nurse LPN will be responsible for providing skilled nursing care to patients in their homes, including administering medications, wound care, and other medical treatments as prescribed by physicians. You will also be responsible for documenting patient care, communicating with healthcare teams, and maintaining accurate records.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including administering medications, wound care, and other medical treatments as prescribed by physicians.
  • Document patient care, including progress notes, medication administration records, and other relevant information.
  • Communicate effectively with healthcare teams, including physicians, therapists, and other healthcare professionals.
  • Maintain accurate and up-to-date patient records, including medical history, treatment plans, and progress notes.
  • Collaborate with patients and their families to develop and implement personalized care plans.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have a strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
Environment:
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
