Casa of Hobart is seeking a skilled Occupational Therapist to join our team in a Skilled Nursing & Long Term Care setting. As an Occupational Therapist, you will play a vital role in helping our residents achieve their highest level of functioning and independence.
Key Responsibilities- Evaluate and treat residents with disorders or neurological deficits, weakness, and paralysis.
- Screen residents for occupational therapy needs, including new admissions and long-term residents.
- Complete all necessary documentation, including initial evaluation forms, weekly progress notes, and monthly recertification forms.
- Attend required facility meetings and in-services, including Medicare meetings and patient staffing.
- Develop programs to assist residents in achieving their highest feasible level of functioning.
- Assist the facility in meeting state, federal, and other certification standards.
- Employ and follow facility infection control policies and procedures.
- Respect and follow OBRA Resident's Rights Guidelines.
- Schedule residents on caseload and attend required in-services and meetings.
- Present yourself in a professional manner, including professional conduct, communication, and dress.
- Licensed in the state of Indiana.
- Ability to work in a Skilled Nursing & Long Term Care setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
