Job Summary

Casa of Hobart is seeking a skilled Occupational Therapist to join our team in a Skilled Nursing & Long Term Care setting. As an Occupational Therapist, you will play a vital role in helping our residents achieve their highest level of functioning and independence.

Key Responsibilities
  • Evaluate and treat residents with disorders or neurological deficits, weakness, and paralysis.
  • Screen residents for occupational therapy needs, including new admissions and long-term residents.
  • Complete all necessary documentation, including initial evaluation forms, weekly progress notes, and monthly recertification forms.
  • Attend required facility meetings and in-services, including Medicare meetings and patient staffing.
  • Develop programs to assist residents in achieving their highest feasible level of functioning.
  • Assist the facility in meeting state, federal, and other certification standards.
  • Employ and follow facility infection control policies and procedures.
  • Respect and follow OBRA Resident's Rights Guidelines.
  • Schedule residents on caseload and attend required in-services and meetings.
  • Present yourself in a professional manner, including professional conduct, communication, and dress.
Requirements
  • Licensed in the state of Indiana.
  • Ability to work in a Skilled Nursing & Long Term Care setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
