Job Summary
We are seeking dedicated Registered Nurses to join our team for admissions and start of care (S.O.C) duties on weekends. As an Admissions Registered Nurse with Home Care Connectors, you will be responsible for admitting and performing start of care for new patients, communicating with collaborating nurses regarding patient care, and maintaining accurate patient medical records.
Responsibilities:
- Admit new patients and perform start of care duties
- Communicate with collaborating nurses to ensure seamless patient care
- Maintain accurate and up-to-date patient medical records
- Work collaboratively with the care team to ensure high-quality patient outcomes
Requirements:
- Currently licensed to practice as a Registered Nurse in the State of Connecticut
- Two years of previous home care experience in nursing
- Ability to build rapport with patients and prioritize tasks effectively
- IV Certified/ IV Hydration
- Time Management and Punctuality
